Nothing. It's a Gothic extravaganza, ticking off all the tropes (secret crazy sibling! Madness in the family! Our heroes put through torture scenarios modelled Edgar Allen Poe! etc.). Not that ACD didn't use melodrama in the Holmes stories for the occasional client, but not for Sherlock Holmes & Relations.
